Accuracy in Oasis and ICD 10 coding has been emphasized since the beginning of Oasis and ICD 10. Now, as we approach PDGM, it is more important to make sure you are getting it right. Now, you may have a great system in place with employees that code but you may not have anything to gauge whether or not it is being done correctly. It is time to do a check and balance for your agency. Make sure you are getting the reimbursement you deserve and the accuracy that has to be there.
What is supposed to be my check and balance?
No matter your current system, agency owners and administrators need to have their current system evaluated. All clinical staff undergo a competency every year and some form of this makes sense in the way of clinical coding. Are your coders certified? Does the outsourcing company you use have only certified coders? Does your coding employee serve other job roles within your organization? Let’s look at how you provide a check and balance within your organization.
Reality is that if your coder wears different hats in your organization, it becomes unrealistic to expect them to maintain excellence in coding. Often, we see coding staff who are also office managers. Over time, keeping up with ICD 10 coding changes and rules becomes less and less the routine of the employee. That employee becomes more focused on the other job roles he or she fills. If you already outsource, make sure coding is being completed by coders certified in coding. It is also important for those coders to be experts in Oasis to code in conjunction with the clinical Oasis data.
First, contact an coding outsource company to ask for a side-by-side comparison. Pick 5 random charts and ask the outsource company to code them at the same time of your current coder. Or, allow the outsource company to pick 5 random charts that have already been coded. You will be able to see if there is any discrepancy between the outsource and the current coding system you have in place. This will let you if you are on the right track or you need to dig more into the discrepancy.
