Washington Client & Employee File Forms Set - NON-SKILLED

Washington State requires that all applicants for licensure of a home care or home health agency must submit a sample employee file and a sample client file. These files are constructed to meet the state requirements. The client file and the employee file samples vary based on whether it is a non-skilled or a skilled agency. Be sure to select the correct set of forms for your application, either Skilled or Non-skilled.
